Ready to claim a Voucher?

If you have already had a Voucher approved, you can still claim it within the timescales detailed in your Funding Agreement document.

When you are ready to claim your voucher, print, complete and sign and post the following to us:

Voucher Claim Form Pack

Remember to also include

  1. Connection Voucher or Innovation Voucher Claim Form
  2. Voucher Redemption Invoice
  3. Output form (P4)

Remember to also include

  1. Original supplier fully itemised invoice (including new speed declaration for connection voucher)
  2. Invoice payment evidence (bank statement/card statement) evidencing payment to the supplier
